No personally identifiable visitor information may be provided to other parties for marketing, advertising, or other uses.
Your consent
Your consent is required to share private health information with anyone other than yourself, unless you are a minor and the information may be shared with your parents or legal guardians. Your consent is required to include identifiable, personal photography or video clips with a form that must be signed by you.
