What is a dermal filler?

As we age, fullness of the face decreases. This leads to a deflated, older appearance. Dermal fillers may be injected into the face in a routine office visit to reduce the appearance of facial deflation and wrinkles. These fillers include hyaluronic acid products such as Restylane, Perlane and Juvederm, which all contain lidocaine for comfort.

Voluma is a particularly exciting injectable that provides excellent volume in the cheek, restoring youthful fullness and secondary eradication of nasolabial folds and jowls. It takes about 20 minutes to treat and individuals leave the office with a “done” result with minimal swelling or bruising. Voluma is also great for improving on remarkable facial asymmetries.

Restylane has been around the longest. It has the smallest molecular composition, and is best for fine lines, like those in the face and lip area, and also in smoothing the junction between the lower eyelid and cheek fullness, with reduction of dark circles under the eyes. Volbella is a newer product that produces more subtle results in the lip that last one year.

Juvederm and Perlane are similar in nature and have larger molecular composition. They are optimal for deeper lines, like the folds between the nose and lips (nasolabial folds), marionette lines, and deep furrows between the brows. Juvederm and Perlane are also excellent for creating fuller lips.

Kybella, a newer injectible, has been designed to combat stubborn, unmanageable submental fat, known colloquially as a “double chin.” Kybella relies on deoxycholic acid molecules, compounds found organically in the human body that work to dissolve fat deposits, releasing them as bodily waste. Once Kybella has been adequately injected into the skin, in at least 2-3 sessions, the upper neck is beautifully contoured and slimmed for a more youthful, attractive appearance.

Another exciting dermal filler on the market is Sculptra. Sculptra works to stimulate lost collagen, a protein responsible for the skin’s suppleness, plumpness and resilience. Sculptra is often a key component in a “liquid” facelift, and is highly versatile when it comes to improving facial contours. Sculptra can also do wonders for the nasolabial folds and sunken, sagging jowls. Sculptra requires 2-3 treatments, and offers more global volumization in the face.

Who is a good candidate for dermal fillers?

Healthy men and women in their 30’s and older who are looking to improve facial fullness and reduce wrinkles are excellent candidates for treatment. Clients should have reasonable expectations for improvement rather than perfection. Fillers are a great way to achieve rejuvenation without any scars or downtime.

During the initial consultation you will discuss with Dr. Shermak what you would like to improve so she can understand your goals and determine how to best achieve your desired result. During that initial visit, the treatment may take place. Depending on the treatment, a second treatment is recommended 6 months to a year later to achieve optimal results.

What is the recovery like after dermal filler treatment?

You may experience temporary swelling, numbness, and bruising that require head elevation, ice packs and massage with Arnica Montana herbal supplementation for several days. Most resume regular activities and look great by the following day.

COVID-19 Update 03/15/2022

This is a new day, with new safety protocols. Our goal is to keep everyone safe and respect the continued concerns COVID-19 may present.

The state of Maryland is now fortunately downgraded to Yellow Status. COVID stress on the medical system has lessened and most areas in the state no longer mandate masks.

Currently in our office we are maintaining sterile surfaces and compulsive cleaning protocols – as always. We are requesting information about vaccination status, to particularly better help us prepare for patients interested in surgical procedures.

We are not mandating masks. We do respect our patients’ wishes and will wear a mask when requested to do so. We want our patients to be absolutely comfortable. All of our staff are vaccinated.

This is certainly an evolving issue and we will continue to pivot as COVID statistics change – hopefully for the better! Thank you!
